Uploading Completed Certification Forms
How do I upload completed medical certifications received from an employee or from a health care provider?
If the employee provides a hard copy of the completed form, or if health care provider faxes the form to your organization directly,
- First, scan the form to your computer.
- Next upload it into the eFMLA system by clicking ‘Upload’ next to the applicable certification in the employee’s EFI Profile.
In the example below, this is the Employee's Serious Health Condition.
If it were for a Family Member's Serious Health Condition, the other option would be available.
Select the file to upload, along with the date the certification was received and click 'Submit'.
The employee will receive an email notification that the certification has been received and will be able to view the certification form by logging in to their Employee Dashboard.
Note: If there is no upload button: If you have not created a certification form, the 'Upload' button option will not appear.
IF there is no upload button, first Click on the “Not Done – Create” button to create the form so that you can access the ‘Upload’ button to upload the certification.
You do not need to email the certification form to the employee unless you require information from the employee’s health care provider regarding any medical work restrictions.
