eFMLA Terms & Definitions

CLTR - Cover Letter - The cover letter is to inform the employee which FMLA Notices, Forms or other documents are being transmitted electronically, as well as to provide other information related to the employee's leave.


EN - Eligibility Notice - This notice is for notifying an employee of their FMLA eligibility status, requesting any needed certification or other documentation related to the employee's leave request, and providing the employee with information regarding their Rights and Responsibilities for taking FMLA leave.


DN - Designation Notice - This form is for informing an employee whether their FMLA leave request has been (a) approved, (b) delayed, or (c) not approved, or whether additional information is needed, as well as to provide additional information regarding the employee's FMLA leave if the FMLA leave is approved.


FFD/RTW - Fitness-for-Duty/Return-to-Work - This certification form is to obtain authorization from an employee's health care provide that an employee is able to resume work with or without any medical work restrictions.


QE - Qualifying Exigency.


SIIS - Serious Illness or Injury of a Servicemember.


ESHC - Employee Serious Health Condition.


FMSHC - Family Member Serious Health Condition.


EFI - Employee FMLA Instance - This is anacronym used throughout eFMLA to indicate one instance of an employee FMLA leave request for a single FMLA-qualifying reason within a single 12-month period based on your organization's 12-month calculation method.


Emp. ID - The employee's identification number.


EFI # - The sequential number of FMLA instances (EFI’s) listed for this employee in your organization's eFMLA account.


Last EFI Date - The date that the latest EFI was initially entered.


What do the X's and checkmarks mean in an employee's EFI Summary?

The symbols in the EFI Summary List indicate which Notices or Forms are complete, incomplete or pending further input:

  • A red ‘X’ signifies that the Notice or Form has not been started or completed.
  • A yellow check mark indicates that the Notice or Form is awaiting input from the employee or the health care provider in order to be completed.
  • A green check mark signifies that the Notice or Form has been completed by all required parties.
